Medical Form

We require medical forms for all Scouts, as well as any parent or family member that comes on an overnight outing. These should be filled out and returned to your Den leader.

Activity Acknowledgement Form

Some activities held by the Lake Erie Council (Cub Haunted, for example) require youth participants to fill out the Activity Acknowledgement Form.
